Home and Community-Based Services is the coordination of a broad range of services arranged in response to a person’s assessed needs and resources by a group of professional care managers.  Once a client is assessed and care plan by the Aging and Disability Resource Center, HCBS takes it over and partners with the client long-term for the duration of the plan and beyond.  It involves person-centered and person-driven care plan development by certified, professional care managers.  Individual goals are explored and prioritized with clients.  Person-centered and driven, the client and care manager develop the best plan to meet the needs of the individual.  Referrals, resources, and support provided are provided allowing for maximal independence within the community.  A multitude of services can be provided to help meet the individual’s needs. This includes, but is not limited to; home-delivered meals, personalized attendant care, structured family caregiving, personal emergency response unit, home and community assistance, and beyond.

Contact the In-Home Services provider with any changes in scheduling and staffing such as to reschedule a day, to request a different caregiver, or general issues. If you are not able to resolve an issue with the In-Home Services provider, or feel the issue is not one that falls within this category, please contact your NWICA Care Manager listed above. Please inform the NWICA Care Manager when you have been admitted to the hospital or if you are not being properly served by the In-Home Service Provider. It is important to have a back-up plan in place in the unlikely event that services are delayed or cannot be provided.

Contact the Field Supervisor is you feel the NWICA Case Manager is not working to resolve any issues you have taken to them for assistance.
